I find it incredibly rewarding to provide care management services for senior adults and offer person centered care planning to help our seniors maintain their quality of life. I spent the first 20-plus years of my career as a registered nurse and gained extensive clinical experience in a team setting. I found my calling when I was recruited to open and manage a branch of a home care company located onsite in a continuing care retirement community (CCRC). As a result, I learned firsthand how vulnerable our senior population is and learned how to provide the individualized assistance and support they need.

It is a privilege to help clients and families devise care plans that illustrate our mission at Senior Care Solutions Chicago: Attaining safety and quality of life for the senior members of the community while preserving as much independence and dignity as possible. We do this by compassionately guiding seniors and their families in the areas of personalized assessments and care planning, coordination of care, utilization of appropriate community resources, and advocacy when needed.

Our Process

At Senior Care Solutions Chicago, we assist families in creating supportive networks in either one’s home or in the senior housing setting through our relationships with trusted providers and by coordinating services tailored to each client’s unique situation.

Care plans: We begin by completing a health assessment that determines each client’s current level of care. A typical assessment with recommendations takes an average of three to six hours, depending on the complexity of each client’s circumstances. We review and explain the care plan and accompanying recommendations so that clients and their families can take an active part in the care planning process.

Plan implementation: A care manager administers the agreed upon health care plan and performs ongoing evaluation.

Ongoing communication: We carefully monitor care plans to ensure each client’s needs are met and make adjustments as needed. We keep you informed every step of the way.

Educating families and their support networks to reduce stress and worry when making challenging decisions about a loved one’s care.

I found the article ''Services for the Elderly Who Don't Want to Move'' [May 9, some regional editions] very empowering in encouraging seniors to remain at home with supportive services. I am disturbed however that the article neglected to discuss the important role of geriatric care managers other than mentioning them as surrogates to absent family members. Geriatric care managers have a multifaceted role. They are skilled professionals who advocate for seniors. They are trained to clinically evaluate the needs of older adults and assist with ongoing monitoring and placement of services (for medical needs as well as safety in the home, or for concrete needs such as house repairs and coordinating visits to doctors or other professionals). They can offer emotional support regarding serious issues of depression, bereavement, retirement and transitions due to physical limitations and life-style adjustment, as well as promote socialization and exercise. As a society we have become fearful of Health Maintenance Organizations that make medical decisions using nonmedical staff, or that are concerned about profits instead of what's in the best interest of the insured. Relationships established with seniors, family members and geriatric care managers should be part of a continuing dialogue to create a safe and comfortable independent life style, not just a service to call when the leaves need collection or the grass needs to be mowed.
